{"id":4074,"date":"2021-11-16T15:31:39","date_gmt":"2021-11-16T20:31:39","guid":{"rendered":"https:\/\/thebao.ca\/?p=4074"},"modified":"2021-11-16T15:31:40","modified_gmt":"2021-11-16T20:31:40","slug":"notice-to-managers-reminder-to-those-who-havent-yet-submitted-alrs","status":"publish","type":"post","link":"https:\/\/thebao.ca\/stage\/notice-to-managers-reminder-to-those-who-havent-yet-submitted-alrs\/","title":{"rendered":"Notice to Managers: Reminder to those who haven\u2019t yet submitted ALRs"},"content":{"rendered":"\n<h6 class=\"wp-block-heading\">&#8212;\u00a0It\u2019s time to submit your Funeral Establishment and<br>Transfer Service Annual Licensure Report <\/h6>\n\n\n\n<p class=\"wp-block-paragraph\">The BAO reminds managers who have not yet submitted their required Annual Licensure Report (ALR) \u2013 for Funeral Establishments and Transfer Services \u2013 to do so soon.<br><br>Funeral Establishment Operator (Class 1 and 2) and Transfer Service Operator (Class 1 and 2) ALRs are due by Dec. 31, 2021.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Download ALR form<\/strong><br><br>The&nbsp;<a href=\"https:\/\/portal.thebao.ca\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>BAO web portal<\/strong><\/a>&nbsp;is open for managers to download your operator renewal form.<br><br>The operator renewal fee is payable by Visa, Mastercard, cheque or money order.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/thebao.ca\/wp-content\/uploads\/Credit-Card-Authorization-Form-2024.pdf\"><strong>Click here for the BAO\u2019s credit card payment form<\/strong><\/a><strong>.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Annual Licensure Renewal can be submitted by:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Email to&nbsp;<a href=\"mailto:Licensing@thebao.ca\" target=\"_blank\" rel=\"noreferrer noopener\">Licensing@thebao.ca<\/a>;<\/li><li>Fax to 647-748-2645; or<\/li><li>Regular mail at &nbsp;<br>Bereavement Authority of Ontario<br>100 Sheppard Ave East, Suite 505<br>Toronto, ON M2N 6N5&nbsp;<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Remember that\u2026&nbsp;<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>The BAO is&nbsp;<a href=\"https:\/\/thebao.ca\/notice-to-funeral-transfer-service-operators-public-health-unit-inspections-not-required-for-licence-renewal-again-this-year\/\" target=\"_blank\" rel=\"noreferrer noopener\">not requiring holding\/embalming room inspections by local public health units for licence renewal purposes<\/a>&nbsp;again this year.<\/li><li>There is no longer a pre-populated Report of Prepaid Funds form.
