Then, by no later than Dec. 31, please submit:&nbsp;<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Your completed renewal form,<\/li>\n\n\n\n<li>Payment, and<\/li>\n\n\n\n<li>The<strong>\u00a0Attestation form for your onsite holding and\/or embalming room<\/strong> is found <a href=\"https:\/\/thebao.ca\/for-professionals\/funeral-transfer-service-professionals\/forms\/\" target=\"_blank\" rel=\"noreferrer noopener\">on this webpage under the Application Forms tab<\/a>. Not sure who is the manager of record? You can search your business on our&nbsp;<\/strong><a href=\"https:\/\/portal.thebao.ca\/public-register\"><strong>public register<\/strong><\/a><strong>&nbsp;to see.<\/strong><br><br><strong>Payment:<\/strong><br>The operator renewal fee is payable by&nbsp;<a href=\"https:\/\/thebao.ca\/wp-content\/uploads\/Credit-Card-Authorization-Form-2024.pdf\">Visa, Mastercard<\/a>, cheque or money order. Fees have gone up this year by 13.6%, which represents an inflationary increase, as per&nbsp;<a href=\"https:\/\/thebao.ca\/notice-to-the-profession-fees-increasing-for-the-2023-renewal-year-first-fee-increase-in-seven-years\/\">a previous BAO notice<\/a>.&nbsp;<br>Paying by cheque? Please include the operator licence number in the memo section.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Late fee:<\/strong><br>Renewals received by email\/fax or postmarked after December 31<sup>st<\/sup>&nbsp;will be subject to a late fee of $227.20.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Attestation form for your onsite holding\/embalming room:<\/strong><br>The BAO is&nbsp;<a href=\"https:\/\/thebao.ca\/notice-to-funeral-transfer-service-operators-public-health-unit-inspections-not-required-for-licence-renewal-again-this-year\/\" target=\"_blank\" rel=\"noreferrer noopener\">no longer requiring holding\/embalming room inspections by local public health units for licence renewal purposes<\/a>. Instead, it is up to each site manager to self-report on their onsite holding\/embalming room(s) from now on, using the&nbsp;<a href=\"https:\/\/thebao.ca\/wp-content\/uploads\/2022\/08\/BAO_AttestationHoldingEmbalmingRoom-2.pdf\">Attestation for Holding\/Embalming Rooms<\/a>&nbsp;form.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Do you manage more than one location?<\/strong><br>If you manage more than one location, you should have received an email from one of our Licensing Officers with your renewal forms attached. If you did not, please contact our Licensing department right away (contact info below).&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Operator wall licences:<\/strong><br>Wall licences are no longer being issued, as they are no longer required to be displayed,&nbsp;<a href=\"https:\/\/thebao.ca\/notice-post-your-price-list-licence-and-link-to-the-bao-consumer-information-guide-on-your-websites\/\">as per a previous BAO notice<\/a>.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Changes to Officers\/Directors\/Shareholders:<\/strong><br>If you are reporting any changes to officers, directors, or shareholders, you will need to provide copies of supporting corporate documentation, as applicable:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Articles of Incorporation \/ Amalgamation \/ Amendment (whichever is most recent)<\/li>\n\n\n\n<li>Officers and Directors registers, or Ontario Form 1 \u2013 Initial Notice \/ Notice of Change re. Officers and Directors, or in the case of a federal corporation, the equivalent federal documents from Industry Canada<\/li>\n\n\n\n<li>Shareholder\u2019s register confirming equity ownership<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Also, any transfer of shares that results in a change of controlling ownership (transfer\/sale of 51% or more of the equity shares) requires a&nbsp;<a href=\"https:\/\/thebao.ca\/the-link-no-longer-exists\/\">business licence application<\/a>.
