Instead, you will receive an email early next week from one of our Licensing Officers with the ALR forms attached. Keep in mind, you will still need to login to the Licensee Portal and renew your personal licence.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Managers of Transfer Service Operator (Restricted) Sites<\/strong><strong>:&nbsp;<\/strong>You will not find your ALR form on the Licensee Portal; instead, it has already been emailed to you by one of our Licensing Officers.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Payment:<\/strong>&nbsp;If your site registered less than 10 deaths between November 1, 2022 and October 31, 2023, your site renewal fee is $300. Otherwise, fees this year are a combination of last year\u2019s fees and the new fees that came into effect on July 1, 2023, as previously advised. Please use our&nbsp;<a href=\"https:\/\/thebao.ca\/wp-content\/uploads\/2023\/04\/FE_TS-Operator-licence-renewal-calculatorNOV30.xlsx\" target=\"_blank\" rel=\"noreferrer noopener\">CALCULATOR<\/a>&nbsp;(a downloadable Microsoft Excel file) to assist you in calculating this year\u2019s renewal fee.&nbsp; Fees are payable by\u202f<a href=\"https:\/\/thebao.ca\/wp-content\/uploads\/Credit-Card-Authorization-Form-2024.pdf\" rel=\"noreferrer noopener\" target=\"_blank\">Visa, Mastercard<\/a>, cheque or money order (include the site licence number(s) in the memo section).&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Late fee:<\/strong>&nbsp;Renewals received by email\/fax or postmarked&nbsp;after January 15, 2024 will be subject to a late fee of $244.47.&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Holding\/Embalming Room Report:<\/strong>&nbsp;Each site manager must self-report on their onsite holding\/embalming room(s) if any, using the&nbsp;<a href=\"https:\/\/thebao.ca\/wp-content\/uploads\/2023\/04\/AttestationHoldingEmbalmingRoomsForm_editable_rev_July3_2023.pdf\" rel=\"noreferrer noopener\" target=\"_blank\">Attestation for Holding\/Embalming Rooms<\/a>&nbsp;form.&nbsp; Please download and complete this form, then submit it with your completed ALR form and payment.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Operator wall licences<\/strong>&nbsp;are no longer being issued, as they are no longer required to be displayed,\u202f<a href=\"https:\/\/thebao.ca\/notice-post-your-price-list-licence-and-link-to-the-bao-consumer-information-guide-on-your-websites\/\" rel=\"noreferrer noopener\" target=\"_blank\">per a 2021 BAO notice<\/a>.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Changes to Officers\/Directors\/Shareholders:&nbsp;<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you are reporting any changes to officers, directors, or shareholders, you will need to provide copies of supporting corporate documentation, as applicable:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Articles of Incorporation \/ Amalgamation \/ Amendment (whichever is most recent)&nbsp;<\/li>\n\n\n\n<li>Officers and Directors registers, or Ontario Form 1 \u2013 Initial Notice \/ Notice of Change re.
