{"id":8341,"date":"2024-09-18T10:46:26","date_gmt":"2024-09-18T14:46:26","guid":{"rendered":"https:\/\/thebao.ca\/?p=8341"},"modified":"2024-09-18T10:46:27","modified_gmt":"2024-09-18T14:46:27","slug":"avis-aux-gestionnaires-rappel-soumettez-vos-alr-detablissement","status":"publish","type":"post","link":"https:\/\/thebao.ca\/stage\/fr\/avis-aux-gestionnaires-rappel-soumettez-vos-alr-detablissement\/","title":{"rendered":"Avis aux gestionnaires : Rappel &#8211; Soumettez vos ALR d&#8217;\u00e9tablissement"},"content":{"rendered":"<h6 class=\"wp-block-heading\">&#8212; Il est temps de soumettre votre rapport annuel sur les licences des \u00e9tablissements fun\u00e9raires et des services de transfert.<br>Service de transfert Rapport annuel de licence<\/h6>\n\n\n<p class=\"wp-block-paragraph\">Le BAO rappelle aux gestionnaires qui n&#8217;ont pas encore soumis leur rapport annuel de licence (ALR) &#8211; pour les \u00e9tablissements fun\u00e9raires et les services de transfert &#8211; de le faire <strong>tr\u00e8s bient\u00f4t<\/strong>.<br><br>Les ALR des op\u00e9rateurs d&#8217;\u00e9tablissements fun\u00e9raires (classes 1 et 2) et des op\u00e9rateurs de services de transfert (classes 1 et 2) doivent \u00eatre remis au plus tard le 31 d\u00e9cembre 2021.<\/p>\n\n\n<p class=\"wp-block-paragraph\">Si vous l&#8217;avez d\u00e9j\u00e0 fait, merci.<\/p>\n\n\n<p class=\"wp-block-paragraph\"><strong>Note \u00e0 l&#8217;attention de ceux qui g\u00e8rent<br>plus d&#8217;un site :<\/strong><\/p>\n\n\n<ul class=\"wp-block-list\"><li>Lorsque vous \u00eates connect\u00e9 au portail de renouvellement, si vous ne voyez pas les formulaires de renouvellement pour tous les sites que vous g\u00e9rez, veuillez demander une copie des formulaires ALR manquants en envoyant un courriel \u00e0 <a href=\"mailto:Licensing@thebao.ca\" target=\"_blank\" rel=\"noreferrer noopener\">Licensing@thebao.ca.<\/a><\/li><li>N&#8217;oubliez pas d&#8217;indiquer votre num\u00e9ro de licence d&#8217;\u00e9tablissement dans le courriel que vous nous adressez.<\/li><\/ul>\n\n\n<p class=\"wp-block-paragraph\"><strong>T\u00e9l\u00e9charger le formulaire ALR<\/strong><br><br>Le <a href=\"https:\/\/portal.thebao.ca\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>portail web BAO<\/strong><\/a> est ouvert aux gestionnaires pour qu&#8217;ils puissent t\u00e9l\u00e9charger le formulaire de renouvellement de l&#8217;op\u00e9rateur.<br><br>Les frais de renouvellement sont payables par Visa, Mastercard, ch\u00e8que ou mandat. <\/p>\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/thebao.ca\/wp-content\/uploads\/Credit-Card-Authorization-Form-2024.pdf\"><strong>Cliquez ici pour obtenir le formulaire de paiement par carte de cr\u00e9dit de la BAO<\/strong><\/a><strong>.<\/strong><\/p>\n\n\n<p class=\"wp-block-paragraph\">Le renouvellement annuel de l&#8217;autorisation d&#8217;exercer peut \u00eatre soumis par :<\/p>\n\n\n<ul class=\"wp-block-list\"><li>Envoyer un courriel \u00e0 <a href=\"mailto:Licensing@thebao.ca\" target=\"_blank\" rel=\"noreferrer noopener\">Licensing@thebao.ca ;<\/a><\/li><li>T\u00e9l\u00e9copie au 647-748-2645 ; ou<\/li><li>Courrier ordinaire \u00e0 l&#8217;adresse suivante  <br>Office des bercements de l&#8217;Ontario<br>100 Sheppard Ave East, Suite 505<br>Toronto, ON M2N 6N5 <\/li><\/ul>\n\n\n<p class=\"wp-block-paragraph\"><strong>Rappelez-vous que&#8230; <\/strong><\/p>\n\n\n<ul class=\"wp-block-list\"><li>Cette ann\u00e9e encore, le BAO <a href=\"https:\/\/thebao.ca\/notice-to-funeral-transfer-service-operators-public-health-unit-inspections-not-required-for-licence-renewal-again-this-year\/\" target=\"_blank\" rel=\"noreferrer noopener\">n&#8217;exige pas que les unit\u00e9s locales de sant\u00e9 publique proc\u00e8dent \u00e0 des inspections des salles d&#8217;attente\/de st\u00e9rilisation pour le renouvellement des licences<\/a>.<\/li><li>Il n&#8217;existe plus de formulaire pr\u00e9-rempli de d\u00e9claration de fonds pr\u00e9pay\u00e9s.
