Licensing Forms for Funeral & Transfer Service Professionals

As announced in our April 16 Notice to the Profession, our new customer relationship management system, the Ontario Bereavement Information System (OBIS), went live on April 30.

With our new system, the applications listed below are now accepted online via our Licensee Portal, which can be accessed by clicking on Licensee Login at the top right of your screen or by using this link: https://portal.thebao.ca/

To submit the above applications via our Licensee Portal, you must have an online account. If you do not yet have an online account, first check your inbox and spam folder to see if you received an email from “OBIS Admin” containing a unique link to create an account. If not, please email your request for an account to PersonalLicensing@TheBAO.ca, and attach an image of your government-issued photo ID. Please also indicate which type of application you intend to submit.

For exam registration and personal licence applications, we must obtain confirmation of completion of required education from the applicable college or association, prior to you being able to submit your application.

*If you already have a personal licence and wish to apply for a licence of a different class, you must have a separate account for each class of licence (for example, one account for a Funeral Preplanner licence and another for a Sales Representative licence). You will need to email PersonalLicensing@TheBAO.ca with a request to create a second account.
