Notice: Cemetery, crematorium and hydrolysis operators can now submit their ALR Form 1 & 2 online

We are pleased to announce the launch of our new online renewal and care and maintenance reporting for cemetery, crematorium and hydrolysis operators. Thank you for your patience while the necessary adjustments to our database were being made.
 
As the main operator contact, you will be responsible for submitting your Annual Licensure Report (ALR) – Form 1 and your Report on Care and Maintenance Fund/Account – Form 2, online through our Licensee Login Portal.
 
The deadline for submitting your Form 1 and Form 2 is three (3) months from your fiscal year end.

Reminder – Renewal fees are waived this year
 
There is no renewal fee to be paid this year by BAO licensees, thanks to the provincial government’s COVID-19 support. The fee waiver only applies to licence renewal fees that were/are due between April 1, 2020 and March 31, 2021, inclusive.
 
To qualify for the fee waiver, you must renew (submit your Form 1) no later than March 31, 2021.
 
Upon renewal of your licence, you will be able to download a new operator licence for each site you operate. The licence need only be displayed in the main entrance of any office that is open to the public.
