Notice to FE-1, FE-2, TSO-1 and TSO-2 Site Managers: The portal is open to renew your BAO site licence

IMPORTANT: PLEASE READ CAREFULLY 

Further to yesterday’s email to all funeral and transfer service personal licensees, the BAO Licensee Portal is also open for you to download your Annual Licensure Report (ALR) form to renew your site licence.   

The deadline to submit your ALR has been extended to Jan. 15, 2024. If you have not already done so, please submit your completed ALR form, payment and the Attestation form for your holding and/or embalming room using one of the following options, in order of preference:   

  • By email to Licensing@TheBAO.ca 
  • By fax to 647-748-2645 
  • By mail or courier to (please keep a copy for your records): 
    Bereavement Authority of Ontario 
    100 Sheppard Ave East, Suite 505 
    Toronto ON  M2N 6N5 

If you manage more than one site: You will not find all your ALR forms on the Licensee Portal. Instead, you will receive an email early next week from one of our Licensing Officers with the ALR forms attached. Keep in mind, you will still need to login to the Licensee Portal and renew your personal licence. 

Managers of Transfer Service Operator (Restricted) SitesYou will not find your ALR form on the Licensee Portal; instead, it has already been emailed to you by one of our Licensing Officers.

Payment: If your site registered less than 10 deaths between November 1, 2022 and October 31, 2023, your site renewal fee is $300. Otherwise, fees this year are a combination of last year’s fees and the new fees that came into effect on July 1, 2023, as previously advised. Please use our CALCULATOR (a downloadable Microsoft Excel file) to assist you in calculating this year’s renewal fee.  Fees are payable by Visa, Mastercard, cheque or money order (include the site licence number(s) in the memo section). 

Late fee: Renewals received by email/fax or postmarked after January 15, 2024 will be subject to a late fee of $244.47.  

Holding/Embalming Room Report: Each site manager must self-report on their onsite holding/embalming room(s) if any, using the Attestation for Holding/Embalming Rooms form.  Please download and complete this form, then submit it with your completed ALR form and payment. 

Operator wall licences are no longer being issued, as they are no longer required to be displayed, per a 2021 BAO notice

Changes to Officers/Directors/Shareholders: 

If you are reporting any changes to officers, directors, or shareholders, you will need to provide copies of supporting corporate documentation, as applicable: 

  • Articles of Incorporation / Amalgamation / Amendment (whichever is most recent) 
  • Officers and Directors registers, or Ontario Form 1 – Initial Notice / Notice of Change re. Officers and Directors, or in the case of a federal corporation, the equivalent federal documents from Industry Canada 
  • Shareholder’s register confirming equity ownership 

Also, any transfer of shares that results in a change of controlling ownership (transfer/sale of 51% or more of the equity shares) requires a business licence application, which can be found on our website here, under Application Forms.
