Notice to the Profession: Updates to Form 2 – Report on Care and Maintenance Fund/Account
The Bereavement Authority of Ontario (BAO) has updated the format of its Form 2 – Report on Care and Maintenance Fund/Account, and the instructions now aligned with the online portal version on OBIS.
Changes to the form include:
- The instructions have been separated from the form itself for ease of use.
- Some form line items have been reorganized and descriptions for each line item have been added for clarity.
- Automated calculations – if you are filling out the form on your computer, you’ll see that the form now features automatic calculations. (You no longer need to calculate every line item as the PDF version of the form now automatically calculates everything for you.)
- A section has been added for audit report submissions for organizations with more than $500,000 in cumulative care and maintenance funds to indicate when the audit report can be submitted.
- For other care and maintenance contributions outside of the requirements, the section has been expanded to include various types of contributions.
- If you have received a consent from the BAO’s Registrar to borrow from your Care and Maintenance Fund, a section has been added under this line item for the borrowing and repayment.
- A section for the reinvestment of interest or dividends back into the Care and Maintenance Fund/Account.
- Please be advised you are now required to submit your price list along with your Form 2. A checkbox has been added at the end of the form as a reminder.
